This form is used for pets who are scheduled for a surgical procedure, which would include not only specialty surgeries, but also spays, neuters, declaws, and growth removals.
If your pet has not been examined at our office, an exam will be required prior to scheduling a surgical appointment.
Please note that vaccinations must be current when your pet is admitted to our hospital.
Please withhold food from your pet after 10 pm the night prior to the surgical procedure.
If your pet is taking medications, please follow the doctor’s instructions for administration on day of the procedure. If medications are given, please note what time the last dose was given.
Patients are admitted on the day of surgery between 7:30-8:00 am. You will meet with a veterinary technician who will answer your questions, and provide you with an estimate for the procedure. Please allow at least 15 to 20 minutes for surgery check in because these appointments are on a first come basis.
